The Melbourne (Vic) Rollover Tanker Exercise organised by the National Bulk Tanker Association (NBTA) at Sandown (Vic) in Melbourne on March 7th was an outstanding success.

With over one hundred registered participants in attendance it demonstrated clearly the demand for this type of information

The instructors from the Transport Industries Skills Centre (TISC) in Canberra Paul Harrison and Roger Van Cornwall competently explained the ins and outs of just what has to be done in a road tanker emergency situation.

TISC are the training providers to CROIERG and conduct our training courses at their first class Sutton Road, Canberra Training Complex. TISC CEO Bob Waldron was also in attendance at Sandown answering any enquiries in regard to the training courses

The newly acquired CROIERG Rollover Tanker Training Unit was the main training aid used for the demonstration. What a great bit of gear!

Rob Perkins Executive Director of the National Bulk Tanker Association (NBTA) put a lot of hours into co-ordinating this Melbourne rollover tanker exercise and it was a really big success. Tony Coutas fulfilled the role of facilitator for the day very capably

This is the latest brochure distributed by the Transport Industries Skills Centre (TUSC) of Canberra on the CROIERG/NBTA Training Programs

For some time now it has been recognised by all stakeholders in the DG Transport industry and emergency services that there has been a need to develop educational programs in relation to dealing with transport emergencies across Australia. Current regulatory requirements set out in Australian Dangerous Goods legislation are very clear about the important obligations on companies that transport dangerous goods.

The National Transport Commission (Model Legislation - Transport of Dangerous Goods by Road or Rail) Regulations 2007 Select Legislative Instrument 2007 No. 319 in clause 14.2.2 in relation to Transport Emergency Response Plan (TERP) states that:

"(1) emergency plan, for the transport of a placard load, means a written plan, for dealing with any dangerous situation arising from the transport of the goods, that is prepared having regard to any guidelines approved by the Australian Transport Council."

(The guidelines amongst other items states that: "The plan should be updated at least annually")

(2) "A prime contractor or rail operator must not transport a placard load if the prime contractor or rail operator does not have an emergency plan for the transport of the goods."

(3) A person must not consign a placard load for transport if the person does not have an emergency plan for the transport of the goods. (14.5, and/2)"

In other words, if you transport dangerous goods YOUR COMPANY MUST HAVE A TRANSPORT EMERGENCY RESPONSE PLAN (TERP)


In response to these legal requirements, the growing concern from emergency services point of view and the industry itself, Canberra Regions Oil Industry Emergency Response Group Inc (CROIERG) and the National Bulk Tanker Association (NBTA) as two of the leading representative bodies in the DG transport industry have in conjunction with the Transport Industries Skills Centre (TISC), developed a competency based program that on successful completion provides for nationally recognised qualifications and formal industry accreditation.

The program is aimed at assisting DG transport companies across Australia meet their legal obligations as well as enhancing their preparedness to respond to transport emergencies whatever the situation. The program has been developed in consultation with DG transport industry representatives, emergency services and industry peak body groups across Australia. The training program has the full support of industry peek bodies, emergency service and The Australasian Fire and Emergency Service Authorities Council (AFAC)

A Stage 2 (Responders) Training Course was held at the TISC Training Complex on the Sutton Road Canberra from March 13th to 15th

Attendees from TransPacific Industries, Kel Campbell Pty Ltd of Port Kembla NSW and AusFuel/Gull from West Australia

Fire & Rescue NSW (FRNSW) also had two officers complete the course as part of our arrangement with FRNSW. This enables the emergency services to work closely with industry personnel and see how industry approach such incidents

A fire pumper and crew, from the Queanbeyan Fire Station, attended on the Friday when the practical sessions were held and having the fire crew in attendance really adds to the realism of the exercise

What happened?

An industrial vacuum truck was vacuuming out a sump in a reagent area of a mineral processing plant when the vacuum pump motor caught fire. The fire then ignited gases in the interceptor (the large receiving tank on back of truck) that had built up to explosive levels. Material was ejected up to 30 m away.

How did it happen? Damaged vacuum tanker

Investigation showed that the fire was started by a wire leading from the battery to the vacuum pump starter motor. The wire was not protected by a fuse and overheated, igniting the wire’s insulation. The resulting fire then ignited gas that had built up in the interceptor. The gas came from the sludge material that was being cleared up by the truck.

The sludge included residue and material from the sodium ethyl xanthate tank in the storage bund.

This material is itself spontaneously combustible, but it can also produce combustible and explosive carbon disulphide gas and ethanol vapors.

Additionally, the vacuum truck was not fitted with any fire suppression other than a hand-held extinguisher


1. Identify any sumps that may contain hydrocarbons or other flammable materials before clean-up work starts and discuss this information with the workers undertaking the task. This information must be input to the risk assessment process used for this type of task at your mine.

2. Ensure that suitable electrical protection techniques are used with such diesel pump and motor installations.

3. Check similar types of equipment and assess whether additional controls, including an appropriate fire suppression system, are needed.

March 7, 2013 Mumbai INDIA

A tanker filled with LPG crashed into safety pillars along the Sion-Panvel highway at Mankhurd around 1.30am on Monday, toppled into a ditch and burst into flames, killing one person. Over a dozen people, mainly residents of shanties nearby, were injured and the key road remained closed for 14 hours.

The blaze left at least seven of the 16 injured critical with over 90% burns.

A bigger tragedy was averted at the busy junction, near Mankhurd station, as traffic was relatively light at that time of night.

The tanker, which had had been filled with 18 tonne of LPG on Sunday afternoon, was en route to Pune at night when the driver lost control. Quaiser Khalid, additional commissioner of police, said: "While the driver was crossing the Mankhurd link road, the tanker collided with the safety pillars and broke them. It then fell into the nullah.

It burst into flames about 20 minutes later. The impact of the explosion flung the driver, who had returned to the tanker after being rescued by passers-by, into the air. He sustained severe burns and is believed to have died.

Eyewitnesses said after the vehicle hit the pillars, the driver, who had seemed drunk, tried to take a sharp turn. He lost control, and the jolt led to the tanker breaking away from the engine and the driver's cab and falling into the ditch. A while later, it burst into flames that spread to the adjacent shanties where residents were asleep.

February 19, 2013 San Antonio Texas USA

A driver was killed when a tanker truck overturned and the truck's cab burst into flames on Interstate 10 on the Far Northwest Side Tuesday morning.

The accident happened around 2:00 a.m. in the westbound lanes of Interstate 10 near Ralph Fair Road. A San Antonio police officer reported hearing a loud boom and seeing flames and then called in the report to the Bexar County Sheriff's Office. Investigators say the driver of the tanker truck apparently lost control, and the truck hit a guardrail and then rolled over. The cab of the truck then burst into flames.

The driver of the tanker truck was killed. According to a preliminary report, investigators believe the driver fell asleep at the wheel.



February 26, 2013 Santa Fe New Mexico USA

Western Refining and its environmental cleanup contractor, D&H Pump, are continuing to work to clean up an estimated 1,600 gallons of gasoline that were spilled from a Western Refining fuel tanker that jack-knifed and ruptured on U.S. 84/285 at the N.M. 599 exit Sunday evening.

The right northbound lane and the right southbound lane of U.S. 84/285 were closed as of 11 a.m. today, Feb. 26.

Traffic was delayed Sunday evening as first responders worked to contain the spill. Motorists are advised that the cleanup work could take several days.

The New Mexico Environment Department’s Hazardous Waste Bureau is working with Western Refining to ensure proper remediation of the site.

The New Mexico Environment Department’s Surface Water Quality Bureau has also been alerted.

NMDOT urges motorists to proceed through the area with caution, watch for traffic control signing, and observe reduced speed limit

A Beaumont man died when the truck-tractor he was driving rolled over in the median of state Highway 185 near Guadalupe Road on Monday.

Traffic on state Highway 185 was diverted at U.S. Highway 59 and Farm-to-Market Road 1686.

By early evening, the highway was re-opened, officials said.

Robert McCann, 27, was trapped in the truck, said Department of Public Safety Trooper Spokesman Gerald Bryant.

He was hauling 175 barrels of crude oil from Shiner to the Port of Victoria when he swerved into the median, overcorrected and rolled over at about 12:20 p.m., Bryant said

A man’s suicide attempt caused a tanker truck accident Friday night that set off a massive fire and shut down part of Interstate 475 for several hours, authorities say.

The accident happened at about 11 p.m. when a vehicle rammed into the side of a fuel tanker exiting the interstate onto Thomaston Road. Lt. Sean DeFoe, Bibb County Sheriff’s Office spokesman, said Nicholas Deonte Owens, 26, of Macon, told deputies at the scene that he drove his 2006 Volvo station wagon into the side of the tanker because he wanted to end his life.

The wreck caused the tanker to burst into flames, which lasted for several hours. DeFoe said the glow could be seen from at least a mile away.

The driver of the tanker truck was unharmed. Owens suffered burned legs, but DeFoe said he walked to the interstate bridge with plans of jumping off until two Macon police officers who happened to be in the area arrived and pulled him down.

The tanker driver, Bryon Cummings of Macon, had just loaded the truck with 7,500 gallons of gas and 500 gallons of diesel fuel and was out to make deliveries. As he exited the interstate, Owens was heading east on Thomaston Road when he saw the truck exiting. DeFoe said Owens accelerated, drove over a curve and plowed into it. Owens told deputies he was upset after an argument with his wife earlier in the evening.

The tanker burst into flames and then a series of explosions ensued, DeFoe said. "It was quite an event out there last night."

As the Macon-Bibb County Fire Department battled the blaze, civilian volunteers with the REACT team were called in to assist with detouring traffic. Repair work was continuing Saturday afternoon, and traffic was still slow on the interstate.

Owens was taken to The Medical Center of Central Georgia, where he was listed in stable condition before being transferred to the August

The deaths of five teenagers in a fiery wreck after their vehicle ran a stop sign and collided with a tanker loaded with fuel affected families throughout the Texas Panhandle, a local official said Tuesday.

"It's had a big effect on the whole area, and I think it always seems more tragic when you have a loss of life when people are so young," said Dr. David Bonner, the mayor pro tem of Dumas. Two of the teens were sisters from Dumas, a town of about 15,000. The others were from a nearby small town.

Authorities say the teens died at the scene. The truck driver, Ezequiel Garcia, was in critical condition Tuesday with burns over 90 percent of his body.

The collision occurred Sunday near Dumas, about 45 miles north of Amarillo. Preliminary reports indicate that alcohol was not a factor, authorities said.

Authorities say Jacob Paul Stipe, 16, of Sunray, was driving when the Chevy pickup went through a stop sign and was hit on the passenger side by the tanker. Area schools are on spring break this week, so news of the tragedy hadn't yet reached many people, Bonner said. "A lot of people are gone," he added.

Texas Department of Public Safety spokesman Juan Medrano said he did not know where the teens were going when the accident occurred. The investigation continues.

The Texas accident was the second nationwide that happened Sunday involving multiple deaths. Early Sunday, five teens and a young woman were killed in an accident in Ohio.

Crews were left with a big mess following a rollover tanker crash in Clearfield County Friday morning.

Officials say the driver fell asleep then lost control along Interstate 80 in Union Township causing the truck to flip onto its roof.

Within minutes crews responded to the wreck and spent hours cleaning up the four thousand gallons of kerosene that spilled.

Two workers were injured in a fire that caused an estimated K29 million in damage at the Premium Petrol Company’s wharf on the Hlaing River in Hlaing Tharyar township on March 12.

The fire started while workers were transferring diesel fuel from a tanker, which had broken down two days earlier, when the nozzle of the hose contacted the tank and sparked.

A Yangon Fire Services department press release said the blaze started at 12pm and was extinguished by 12:55pm.

A total of 348 firemen and 243 assistants, 70 Red Cross workers, as well as 58 fire engines, three support vehicles and one firefighting boat worked to bring the fire under control.

One vessel, the Myit That Oo, was gutted in the fire, while two 2800-gallon tankers, one diesel tank and two work barracks were also destroyed.

Two people were injured and taken to Yangon General Hospital, the press release said.

Workers Ko Lin Htike Aung was charged under section of 285 of the Penal Code, concerning negligent conduct in regards to fire. If found guilty, Ko Lin Htike Aung could face three years’ imprisonment and/or a fine.

A Santa Fe jury this week awarded the family of a Carlsbad man $58.5 million – believed to be one of the largest awards in New Mexico history – in a lawsuit against companies that operated the tanker truck involved in a fatal crash three years ago.

Santa Fe attorney Bill Robins, who represented the family and estate of Kevin Udy, said the award sends a "strong message for safety."

"This was a really clear message to the trucking industry and the oil and gas industry in particular that companies that choose not to follow safety rules will be held responsible," said Robins.

The suit, heard this week in a trial before District Judge Raymond Ortiz, was brought against a trio of companies involved in the transportation of water that had been separated from oil, Robins said.

According to news stories at the time, the tanker truck made a left turn in front of a pickup truck driven by Udy, which then struck the tanker truck’s trailer.

The companies that were sued, along with the truck driver, were Standard E&S LLC and Bergstein Enterprises Ltd., both of Texas, and Zia Transport Inc. of Hobbs.

The defense attorney in the case, Randal W. Roberts of Albuquerque, declined to comment on the case except to say he didn’t think the Wednesday verdict was "supportable" and that the defense would appeal.

The Carlsbad Current-Argus reported in 2010 that Udy, 46, died following the crash on U.S. 62-180. Udy was driving a pickup on the highway east of Carlsbad when the tanker truck made a left turn into the path of Udy’s pickup.

Udy was married and had five children and a stepson. His obituary states that he got his degree in mechanical engineering from the University of Washington and was working in Carlsbad with Intrepid Potash.

On Thursday, Robins told the Journal that the tanker truck’s driver was finishing up a 13-hour workday and was driving an empty 18-wheel tanker truck owned by Zia (a company owned by Bergstein) and leased to Standard E&S.

The estate’s complaint states that the driver, who was hired by Zia, made a "sudden and unexpected" left turn across Udy’s lane, putting him in the way of Udy’s truck, causing Udy to hit the back of the big rig. Udy died en route to the hospital.

Robins said the award could be the largest ever in New Mexico. A call to the state Administrative Office of the Courts on that issue was not returned.

Robins, in an interview Thursday, maintained that the driver had inadequate training and experience to operate the tanker truck. He said the truck was improperly maintained and had a faulty tire and wasn’t roadworthy. Robins also said Standard had a policy in which drivers were not paid hourly, but rather paid by the job.

He said drivers were allocated only a certain amount of time to get a job done, meaning in order to meet deadlines, they had to hurry. He alleged that the company’s drivers had many speeding tickets and said he presented expert testimony to show that the driver was inexperienced and was likely fatigued when the crash occurred.

The verdict in the case was unanimous, according to a partial transcript of Wednesday’s proceedings forwarded by Robins. The court allowed the jury foreman to read a statement regarding the verdict.

"Our hope is that our judgment will clearly communicate that we expect a much higher standard of safety and training in the trucking industry," the jury statement said.
